1. Self attested copy of birth certificate of the child
2. Self attested copy of Aadhar card of the child
3. One passport size photo of the child
4. Self attested copy of Aadhar card of the parents
5. Self attested copy of residence proof (any one)

   • Voter ID Card
   • Current Electricity Bill
   • Passport
   • Domicile Certificate of the child or the parents
